About The Role

The Care Coordinator plays a vital clinical role within Vitasora Health's remote care team, serving as the primary point of contact for patients enrolled in CCM and RPM programs. This position combines clinical expertise with advanced patient engagement skills, including motivational interviewing and health coaching, to foster meaningful relationships that support behavior change and enhance health outcomes. The Care Coordinator is responsible for managing ongoing patient needs, facilitating communication among multidisciplinary care teams, and ensuring compliance with healthcare regulations. This role requires a compassionate, proactive individual capable of addressing social determinants of health, navigating complex cases, and advocating for patients in a dynamic, technology-enabled environment. By delivering personalized, culturally responsive care, the Care Coordinator helps patients achieve their health goals while supporting the overall success of Vitasora's innovative healthcare model.

Qualifications

  • Active and unrestricted Licensed Practical Nurse (LPN), Licensed Vocational Nurse (LVN), or Registered Nurse (RN) license in Hawaii.
  • Minimum of 2 years of clinical experience in chronic care management, telehealth, primary care, or related fields; 5+ years preferred.
  • Experience applying motivational interviewing (MI) and/or health coaching techniques in clinical or patient engagement settings.
  • Proficiency with EHR systems, telehealth platforms, and remote patient monitoring tools.
  • Strong verbal and written communication skills, with the ability to empathetically engage patients and promote behavior change.
  • Knowledge of CMS CCM guidelines, HIPAA regulations, NCQA standards, and healthcare compliance requirements.

Responsibilities

  • Lead monthly outreach calls with patients, utilizing motivational interviewing to encourage behavior change and support health goals.
  • Guide patients through personalized care planning, goal-setting, and accountability strategies using evidence-based coaching techniques.
  • Provide culturally responsive health education and self-management support tailored to individual patient needs.
  • Address social determinants of health by connecting patients with community resources and removing barriers to care, especially in underserved and rural populations.
  • Act as a liaison between patients and their care teams, ensuring seamless communication and coordinated care delivery.
  • Monitor patient progress through RPM data and regular interactions, escalating clinical concerns following structured protocols.
  • Participate in team meetings, case reviews, and care planning sessions to optimize patient outcomes.
  • Maintain accurate, HIPAA-compliant documentation of all patient interactions and care plans within the electronic health record (EHR).
  • Ensure adherence to CMS, NCQA, and Joint Commission standards, and contribute to program performance metrics.
  • Advocate for patients by connecting them with community resources and ensuring their preferences are incorporated into care decisions.
