Role Overview

Our valued partner, is seeking a RPM / RTM / CCM Care Coordinator to support patients with chronic conditions in a remote environment. The ideal candidate will monitor health data, coordinate care plans, engage patients through regular communication, and ensure compliance with program requirements.

This role requires strong English communication skills, meticulous attention to detail, and the ability to work effectively with providers, patients, and care teams. Candidates must possess a relevant healthcare education background and prior experience navigating specialized telehealth and electronic health record tools.

Key Responsibilities

1. Remote Patient Monitoring (RPM)

  • Monitor patient-generated health data from connected devices, including blood pressure monitors, glucometers, pulse oximeters, and weight scales.
  • Review, analyze, and document vital signs and patient health trends.
  • Promptly escalate abnormal readings to healthcare providers according to established protocols.

2. Remote Therapeutic Monitoring (RTM)

  • Track patient adherence to prescribed therapy plans and specialized treatment programs.
  • Monitor therapy-related data and patient-reported outcomes.
  • Conduct regular check-ins with patients to assess treatment progress and identify barriers to adherence, communicating all findings back to providers.

3. Chronic Care Management (CCM)

  • Coordinate care for patients managing multiple chronic medical conditions.
  • Develop, update, and maintain comprehensive, personalized patient care plans.
  • Perform monthly patient outreach and wellness check-ins while educating patients on disease management and preventive care.

Requirements

  • Experience & Education: Healthcare education background required (e.g., MBBS, BDS, Nursing, Pharmacy, Physical Therapy, or related clinical fields). Prior experience working in RPM, RTM, or CCM programs is highly preferred.
  • US Healthcare Familiarity: Experience communicating directly with patients in a U.S. healthcare setting, alongside familiarity with CMS guidelines, chronic disease management, and telehealth workflows.
  • Technical Skills: Proficiency with electronic health record (EHR/EMR) systems and dedicated medical communication platforms, specifically OncoEMR and Doximity.
  • Core Competencies: Strong English verbal and written communication skills, active listening, empathy, critical thinking, excellent time management, and a patient-centered approach.
