2. About the Opportunity:

The Patient Care Coordinator role is responsible for supporting ambulatory case management by assisting in the coordination of healthcare services for a managed care population. This position facilitates communication and resource management to ensure quality, cost-effective care, playing a key role in the delivery of patient-centered support.

3. Responsibilities:

• Provide administrative support to ambulatory case management (ACM) nursing staff

• Enter data related to patient admissions and discharges for ACM programs

• Create, maintain, and process system reports as required

• Organize and maintain filing systems including retrieval and purging

• Serve as initial contact and resource for all ACM programs

• Assist with discharge planning coordination under ACM nurse direction

• Communicate with members, families, or caregivers as directed

• Maintain confidentiality of member information and organizational business

• Assist with referrals to appropriate health plans and disease management programs

• Act as a resource for department and inter-departmental staff and healthcare teams

• Represent the organization professionally

• Comply with all organizational policies and procedures

• Perform additional tasks assigned by ACM nurses, supervisors, and managers

4. Requirements:

• High School Diploma or GED, or equivalent work experience

• Minimum 18 years of age

• At least 2 years of customer service experience, including managing lengthy phone interactions

• Proficiency with Windows-based programs such as Microsoft Word, Excel, Outlook, and Teams

• Ability to work full-time Monday through Friday with flexible 8-hour shifts within 7:30 am to 5:00 pm PST; occasional overtime may be required

• Preferred: 2+ years in medical back office or healthcare field

• Preferred: Knowledge of managed care, medical terminology, third-party payers, regulatory requirements, and government programs

• Ability to maintain confidentiality and secure sensitive documents while telecommuting

• Dedicated, private work area with high-speed internet approved by the organization

• Strong communication, critical thinking, collaboration, and multitasking skills

5. Pay Range and Compensation Package:

• Hourly pay ranges from $18.00 to $32.00 based on full-time employment

• Compensation depends on factors such as local labor market, education, experience, and certifications

• Benefits include a comprehensive package, incentive and recognition programs, equity stock purchase, and 401(k) contribution, subject to eligibility
